Wrexham wellbeing support: residents can refer themselves from age 25

Wrexham residents aged 25 and over can contact the Rainbow Foundation directly for community wellbeing support, without waiting for a GP referral. The council’s new county-wide contract began on 1 September 2026.

The service helps people find practical support, local groups and activities suited to what matters to them. The council identifies loneliness, caring responsibilities, financial pressures, mobility and confidence among the issues that can affect someone’s wellbeing.

How to ask for support

Email spt@rainbowfndn.org.uk or call 01948 830730 to ask about the Social Prescribing and Community Wellness Service. Family members and carers can also make a referral, with the person’s consent. GP practices, health professionals, social care teams and community organisations can refer people too.

The council and Betsi Cadwaladr University Health Board are funding the service. It combines social prescribing with community wellness support and builds on Wrexham’s Community Agent programme, with the Rainbow Foundation awarded the contract after a procurement process.

The practical difference for someone seeking help is that a clinical referral is not the entry requirement. They can start by contacting the service themselves and discussing the support they are looking for. The council’s announcement does not give a waiting time or promise access to a particular activity.
